Enrolment

Amity Health Centre is not currently accepting new patients for enrolment. If this changes, we will update this page.

New patients are required to attend a New Patient Appointment. 

• Adults have a 30 minute consultation, consisting of 15 minutes with the practice nurse, and 15 minutes with the doctor.
  This is a good opportunity to get to know your new health care team, make sure your health screening is up-to-date, and address any current health concerns.
• Children usually have a standard 15 minute appointment with the doctor.

The New Patient Appointment is particularly valuable for those with multiple health issues. If you think you will need longer, please let the receptionist know when you make your appointment.

To complete your enrolment you will need to fill in an enrolment form, and bring it to the practice, together with:

• Either your passport with current visa
• Or your birth certificate and photo ID (e.g. Driver's Licence)
